という意味ですが、この表現は日本語でも

what he is thinking は、「〜こと、もの」の意味の what を用いた文で、「彼が考えていること」という意味です。
ですので、英訳例は「彼が考えていることが[理解できない](https://eikaiwa.dmm.com/uknow/questions/59392/)」という意味になります。
「理解できない」は、英語では普通 do not understand を使います。
日本語につられて can't understand にしてしてまいがちですが、can't を用いると、「理解する能力がない」というニュアンスになってしまいますので、ご注意くださいね。
