母親になら友達に言えないような悩み事も相談できるのってなんて言えばいいのでしょうか。

The first phrase, "This is a family matter" indicates that the only people you can talk to that particular subject are members of your family i.e. mother, father or siblings. The second phrase is more specific, "my mother is the only one I can talk to about this." tells the person you are talking to that you are only comfortable discussing the topic with your Mother.
