日にちが不明な場合は月までを書いてくださいって英語でなんて言うの？

年月日を申請フォームに書かなければいけないのに、日まで具体的にわからない場合は月までを書いて欲しい

date で「日にち」
month で「月」
year で「年」
If you're not sure about ... で「…がわからないなら・定かでないなら」

ーYou don't have to write the date if you don't know it.
「日にちがわからなければ書かなくていいですよ」
If you don't know ... で「…がわからないなら」
to write the date で「日にちを書く」
