If a mistake occurs that could have knowingly been avoided then we would refer to it as a 'careless' or 'needless' mistake/error.
Careless tends to imply that no care was taken to avoid or prevent a situation whereas 'needless' tends to imply that the situation/mistake didn't need to have happened.
