Each time a person has been to a place where him/her does not live, it is called a 'visit'. So, each time he/she has been to Japan, he/she has been visiting Japan. So, this question is related to the number of times he/she has visited Japan. So, to find out the number of times he/she has visited Japan, you may as:

When we ask about the number of visits someone has made, we want to ask them how many times they have been to our country or city. This lets them know that we want them to give us a number as an answer, and we want to know the number of times they have visited.
訪れた回数について聞く場合には、how many times they have been to our country or city（国や都市に何度来たことがあるか）と聞きます。数字で回答してほしいことが相手にわかりますし、これまでに相手が訪れた回数がわかります。

The first example sentence would be appropriate if the questioner does not know whether the visitor has visited japan previously.
The second example assumes the visitor has visited Japan before and just wants to know how many time.
When you ask "how often do you come to Japan?"you are asking about the frequency of their visits to Japan. If you want to ask for the exact number of times they have been to Japan, you can ask:
-How many times have you been to Japan?
-How many times have you visited Japan?

If you want to ask someone who has been to Japan how often they visit you can ask, "How many times have you been to Japan?" or you can ask it in a different way like, "How many times have you traveled to Japan?" You're asking the same question but just in a different way.

または、

You would ask someone you know has visited Japan at least once this question. The person may live abroad and told you that he/she visited Japan. Or, maybe you meet a foreigner in Japan and you want to know if he/she has been to Japan more than once. You would not ask a Japanese person who already lives in Japan.
