「何月に生まれましたか」

・be born
born は bear の過去分詞で、元々は「〜を産む」という意味の受動態です。
この受動態を日本語にすると不自然なので be born で「生まれる」とするのですが、出来事としては過去のことなので、be動詞を過去形にします。
・何月？
3つの例を示しましたが、どれも正しいです。これを理解するには例えば「5月生まれです」を元にして考えるのがいいでしょう。
You were born in May.「あなたは5月に生まれました」
何月か分からない場合は May の部分が what month になりますが、疑問詞は文頭に出すので
What month were you born in?「何月に生まれましたか」
となります。しかし前置詞は疑問詞とセットにして文頭に置いても良いので、
In what month were you born?
と聞くこともできます。この表現の仕方は少しかたく感じられます。
また前置詞を言わずに、
What month were you born?
と言ってもOKです。これは前置詞がなくても意味が明らかだからです。

最後の例は「月」を聞いているわけではないですが、誕生日を聞くときの一般的な表現です。

If you want to ask someone the month they were born then you would ask 'what month were you born?' or 'what month were you born in?' your birthday also tells the person what day/month/year you were born so you could also ask 'when is your birthday?'
人の生まれた月を尋ねたいなら、

You can either ask specifically 'Which/what month were you born in?' or you can just ask the general question 'When is your birthday'.
Both ways will give you the month.

'Which/what month were you born in?'（何月生まれですか）
と尋ねてもいいですし、あるいは一般的に
'When is your birthday?'（誕生日はいつですか）
と尋ねることもできます。
どちらでも何月か分かります。

Person 1- "Hello, I was just wondering. Which month were you born in?"
Person 2- "I was born in January."
