The above sentences mean that you are politely asking them to repeat the name for me. The last sentence can be used to ask them to write it down or spell it for you just in case you finding it hard to get the name. Sometime it helps if you see the name then it will easier to remember.

I usually recommend always use "could" in front of all questions because it makes the sentence more polite. But that's okay to use the first sentence too.
