無難な返答ですが、①②ともに使いやすいと思います。
私もいつも①か②で答えています(笑)
busy = 忙しい
doing well = 元気にしている
他には以下のような表現も使うことができます：
same as always = いつもどおり
feeling a bit down = 少し元気がない
これはよくある質問のひとつです。
端的に言って、特に「これがもっとも好ましい」という返答はありません。
What have you been up to? 自体が具体的に何を意味しているかというと、What have you been doing?「[最近](https://eikaiwa.dmm.com/uknow/questions/36239/)は何（で[忙しく](https://eikaiwa.dmm.com/uknow/questions/36525/)）してたの？」ということです。そのため返事は基本的には「何をしていたのか」で答えることになります。
しかしながら、ただの挨拶程度に言う場合が多く、聞く側も大真面目な返答を求めていないことが多いです。そのため返答もあまり詳細に何かについて答える必要はありません。
以下に返答の種類を3種類挙げておきましたので、ぜひ読んでください。
★ 簡単に
・Not/Nothing much.「特に何も大きなことはないよ」
こんな感じで、半ばはぐらかすような返答もよくされます。そして以下の2種類でもそうですが、ついでに What about you?「君はどう？」のように続けて言って、相手に話を移すこともよくあります。またはまったく別の話題にすぐに切り替えることもあります。
★ 少し具体的に
・I've been studying hard.「勉強頑張ってたよ」
I've been working on my presentation.「プレゼンの準備をしていたんだ」
このように、実際に最近やってきたことを言ってもOKです。
★ 過去の出来事
・I went camping last week.「先週キャンプに行ったよ」
現在完了進行形を使わずに、過去に一度やったことを言って返答にしてもOKです。それぐらいぼんやりした質問であることが分かっていただけると思います。
その他、挨拶とは別に、イタズラをしていた子供などに「あ、何をしてたのかなぁ？」という感じで What have you been up to? を what と you を強調するように言って聞くこともあります。そんなときは Nothing.「何もしてない」と返事しちゃいますよね。

Not much, how about you? - This often is used when you want to keep the conversation brief or you don't feel like elaborating about your life. This is an extremely common polite exchange that happens when you want to acknowledge and show you care about someone without having a in depth conversation. Sometimes it is used as a greeting to friends.
I have been studying English and I recently visited my Grandparents. What have you been doing? - You can insert any subject in place of "English" and in place of "Grandparents". This is when you actually want to tell someone more about yourself and update them about your life/make conversation. Another example could be "I have been studying Science and I recently went to Kyoto for the weekend"
I've been busy with life, same old same old. What about you? - A polite way of telling someone nothing has changed since the last time you talked. "Same old same old" is an idiom that is commonly used when nothing has changed or remains boring.
Not much, how about you?

会話を短くしたかったり、今の状況についてあまり詳しく言いたくないときによく使われます。このやり取りはすごく一般的で丁寧です。ちょっとした会話で、相手を気にかけていることを表します。また、友達へのあいさつとして使われることもあります。

〔訳〕英語の勉強をしたり、最近は祖父母に会いに行ったよ。そっちは？
「English」「Grandparents」の代わりに別のものを入れてもオッケーです。これは、自分の近況について、もっと話をしたいときに使います。
別の例としては：
"I have been studying Science and I recently went to Kyoto for the weekend"
〔訳〕理科の勉強したり、最近週末に京都に行ったよ。
I've been busy with life, same old same old. What about you?

相手と最後に話したときから何も変わっていないと丁寧に伝える言い方です。「Same old same old」は、何も変化がなかったり退屈していることを表すイディオムです。よく使われます。
You can answer the question "what have you been up to" in a million different ways, depending on what you have actually been 'up to' (been doing in general/in life)! In casual conversation many people do not go into detail when talking about themselves unless better acquainted with the person. This is why many of the responses listed in this answer are vague and lacking more detailed information.
"Not much/not a lot" = nothing of note/importance
"to be honest" = to be truthful
"Yourself?" = and you?
「What have you been up to?」の答え方は、あなたがどんな風に過ごしたかによって、無数にあります。
カジュアルな会話では、相手とすごく仲が良くない限り、あまり事細かく伝えたりはしません。

"Not much/not a lot" = 特に何も。

The two sentences give two different answers. You would use the first sentence if you have been very busy. Sometimes in English instead of saying very(in the form of an adverb) we will say crazy. The second sentence is if you have not been busy. We use the term same old same old in English to denote that we haven't been up to anything new.

By saying "nothing much really" you are expressing that
you have not been very busy recently, with nothing
interesting at least!
By adding "how about you?" you are encouraging the
other person to engage in a conversation.

"What have you been up to lately?", is a question to use when you want to know what a person has been doing since you last saw them (which may have been for a long time).
"What have you been up to lately?"とは、その人に最後にあったときから（だいぶたっているので）、最近どうしていたか知りたいときに尋ねるフレーズです。
The first thing to understand is that not all greetings in the form of a question require an exact answer. So when someone says "What have you been up to?" They dont always want an in depth reply, it could just be their preferred greeting.
Personally I would only give a substantial reply to someone I knew well and I knew that they were genuinely interested in my life.
In all other situations it is perfectly acceptable to give a standard reply like "Nothing out of the ordinary."
If you dont want to give any information about yourself you can simply say "This 'n' that." This just means I have been doing things but nothing special to mention.

"What have you been up to?" は
「最近どうしてたの」と「最近何してるの？」や「なんか変わったことある？」
といったニュアンスのある表現です。
なので、これを聞かれた時に最近何か新しく始めたことや近況などがあればそれを普通に伝えるのも回答として好ましいと思います。
2) (なに、相変わらずで... 仕事で忙しくしてるよ。)
"the same old things" は 「代わり映えのしない」や「相変わらず」といった意味で使われる表現です。
変わったことが特に何もない時や話をあまり広げたくない時に使えるフレーズです。
3) (特に何も(楽しいこと)ないな。)
そう喜ばしかったり楽しかったり触れるほどの内容がさしてない時に使える表現です。"nothing much (特にないな)" とも言えます。
この2つの表現は "what's up?" "how are you?" "how have you been?" などに対する回答としても使える便利なフレーズです。
