1) How many floors is your house?
「あなたの家は何階建てですか？」
または、How many stories is your house? とも言えます。
建物の「階」は floor や story を使って言います。
Do you live in a house or an apartment?
「家に住んでる？それともアパート？」
このように聞いても良いですね。
2) My apartment is on the third floor of a five story building.
「私の部屋は5階建ての建物の3階です。」
または
I live on the third floor of a five story building.
「5階建ての建物の3階に住んでいます。」
とも言えます。
on the third floor of a five story building で「5階建ての（建物の）3階」という言い方です。
日本語では、特にアパートに住んでいる場合「部屋」と言うので、英語で room を使いがちですが、英語では apartment と言うのが正しいです。

私の地元（アメリカのミシガン州）では家の話ではfloorよりstoryの方が使われています。意味が同じですが、同じ文で「階建て」と「階」を使ったらfloorを二回使うことよりstoryとfloorを使った方が自然です。
例文：How many stories is your house? My room is on the second floor of a three-story building.
