When someone makes a mistake on live TV, it may be called a 'boob,' 'blunder' or 'gaffe.' There are many TV programmes dedicated to just showing a series of TV gaffes. It has also become customary these days to collect such gaffes during the course of filming - and releasing them later with the production - or perhaps later.
These are all words that can be used for a mistake or an accident.

Words like mistake or mishap can be applied in this situation but the most common word for a mistake that happens during a TV show is a blooper.
Some shows may even make hilarious blooper reels containing all the mistakes that occurred during the filming process, most common for comedy movies.
A mistake that is made specifically in film or TV is known as a blooper. Sometimes at the end of movies they will show bloopers that happened during filming.
You could also say there was a blunder. A blunder is a mistake made through stupidity or carelessness. It is not specific to TV but is a good word to describe this sort of mistake.
