「放送事故」を辞書で調べたらbroadcast accidentが出るかもしれませんが、
日本でしか使われていない気がします。
アメリカのテレビなどでは、「blooper」という言葉がよく使われます。
blooperは生放送の「とちり」という意味で、生放送中に起こったらlive blooperとなります。
放送の種類によって、TV、radio、newsなど一緒に使えます。

You could also say there was a blunder. A blunder is a mistake made through stupidity or carelessness. It is not specific to TV but is a good word to describe this sort of mistake.
