まだ薬が効いてるかは感じられてないって英語でなんて言うの？

この場合、次のような言い方ができますよ。

ーI can't tell if the medicine is working yet or not.
「薬が効いているかどうかまだわからない」
to work で「薬などが効き目がある」

ーI can't tell if this medicine is having any positive effects on me or not.
「薬が私に良い影響を与えているかどうかわからない」
to have positive effects で「好ましい効果がある」

このように I can't tell if ... を使って表現することができます。
