日本のように丁寧にフォークなどをくれるお店は海外ではなかなか見かけないですよね。
くれるかどうかはわかりませんが、Can I have a plastic fork?と聞けばプラスチック製（使い捨ての）フォークをくれると思います。
以前、わたしも似たような状況でスプーンがあるか聞いたところ、お客さん用のものが無かったらしく、従業員用の休憩室に買い置きしてあるスプーンをいただいたことがあります。

日本では当然のようについてくるカトラリー類ですが、海外だとなかなかついてきません。私のいたハワイでは、日本文化も浸透しているので割り箸は基本的にはついていました。が！ローカルのお店だと、手づかみで食べる可能性を考えて、上記のように聞くこともしばしばありました。お箸の国の人には手づかみはつらいですよね。
お客様は神様だ、という言葉にもあるように日本では当たり前。でも国によっては、「つけないのが悪いんじゃなくて聞かないのが悪い」的な発想もまた当たり前にあります。日本とは違い、「これありますか？」「○○をください」と口にださないともらえないのが当然の場所もあります。そのお店に慣れてくると、"I need a fork, just in case (you don't give me one)."「フォーク要ります、念のため（あなたがつけてくれないかもだから）」と言う場合もあります。

Both are polite ways for asking for a fork to eat your meal you may want to check to see if they have any forks first then you would say "do you have any plastic forks please"

The verb 'can' is used because we wish to ask if it is possible. If we already know it is possible to obtain a fork with our takeout meal, we can ask politely for permission to get a fork by using 'May'.
