How do you spell that?は名前を教えて貰ったあとで、結構カジュアルな感じで使うフレーズです。

カジュアルから接客で使っちゃダメではありません！アメリカの接客はとてもフレンドリーで、お客さんと友達のように話します。ですからこのフレーズも接客のときに使います。

このフレーズは前のフレーズよりちょっと明確で丁寧です。
May I ask how you spell your name?
最後のフレーズはとてもフォーマルなフレーズです。
綴りを教えてもあるように頼む許可を頼むから、かなり間接的でフォーマルなのです。
日常的な場面で使ったらおかしいです。ホテルのような場面以外は使いません。

Without it, the request almost becomes a command.
You can say :

Would you please spell your name?

Would you please spell your name?お名前のスペルをいただけませんか？
How do you spell your name? - a straight question to ask someone's name spelling. Could you please spell your name? - a more polite way of asking due to the use of the word "could" but both of them practically mean the same
How do you spell your name?（あなたの名前はどうつづりますか）
→相手の名前のつづりを尋ねています。直接的な言い方です。
Could you please spell your name?（名前のつづりを教えてもらえますか）
→ここでは "could" を使って、より丁寧に尋ねています。
この二つはほとんど同じ意味です。
