丁寧に、低姿勢で聞くならばＹｕｋａ先生のアドバイスの通り、また ”May” と 〝Can” を使い分けてフォーマルさを図ります。
同時に、もしこれが日本で、又は普通はこういった日本語の聞き方をそのまま英語にした自然な聞き方を学んで使えるようになることがまず最初の第一歩になると思います。
さて、では本日はここで一つ、超アドバンスなワンポイントを紹介しますね：
アメリカなどの英語圏の国では、例え私たちがどんなに流暢に英語を使っても、どんなに彼らと同じネイティブさで話しても、現実は残念ながらそこには〝アジア人” である私たちには必ず「差別」というものがあります。
常に謙虚でいること、低姿勢で人に接することはとても良いことに間違いはありませんが、同時に元々これが他の人種より一倍にも二倍にも普通の感覚となっている私たち日本人にとっては、時にやり過ぎる場合があるんですね。
「英語を使うということは日本語を話さない人とのコミュニケーション」となるため、日本を出て世界で使う英語ということになれば私たち日本人にとって常識的な人との接し方もまた変わって、〝変えて” いく必要もでてきます。
〝現地のネイティブ” らもしない、〝下手に謙虚すぎたり”、〝低姿勢すぎたり” することを避け、彼に合わせた同じ言い方、表現を使うことで初めて彼らと同等のレベル、同じ人間であることを態度で示すことも時には必要、結果的に〝こっちから差別をとっぱらう” ことができるようになるんです。
この場合、レストランなどでもまたこれが完全に適応できる一つの場となります。
それは、元々ここで使う英語の単語の由来、主旨、原点に合わせて表現することになります。
元々の意味と由来：
order ： 命令、
service ： 仕えること
serve : （主人に）仕える
waiter : （命令を）待つ人
これらの単語は現代になると全て〝意訳” されるようになっていますが、英語圏では本来の聞き方、本来の表現方法がいまだに一般的に使われます。それは決して〝お願い” するのではなく、〝命令” に近い形で表現することなんですね。
例えば：
I need ice!
（他： I need a menu. （メニューを）
You can bring my coffee now. （コーヒーを持ってきてもいいですよ。）
Will you take my dishes away? （皿を片付けて。））
現にこういった形でお願いすることで淡々と良いサービスを提供してくれることが増えるはず。私たち日本人はちょっと苦手？ 勇気が必要、エラそうにできない、と思ってしまうかもしれませんが、でもだからこそ、それが〝日本語ではない” 、〝英語” の世界なんです。
まずはこういった点を少しでも頭に入れて理解することが大きな第一歩になることと思います。

To keep things as simple as possible, you always have the option of requesting what you want by simply saying the noun you want and adding please. This is more common with some things, (ex: Check please!) but it's easy to understand in nearly any context. (ex: That, please! Steak, please!)

Can I have～？ Could you give me～？という表現があります。
Could you～？とするとより丁寧になります。
友達と話していて、氷ちょうだい！と言っているなら
I want some ice!と言っても良いですし。

The easiest way to say this would be "Can I have ice with my whiskey please?". It is quite polite and very common. It can be used in formal or informal situations like at a bar, asking a bartender or a friend at his or her house.
"I can't drink whiskey without ice, can you add some please?". Again this phrase is very common and if you say it like this it sounds very polite and friendly. You already mentioned that you can't drink whiskey without ice, so you don't have to mention the word again.

These are three different ways that you can use interchangeably to ask the same question or make the same request. The biggest difference between the three is that the final example is using more of a commanding imperative form to request for ask, where as the other two might be more formal and polite.
If you would like to ask for a cup of ice, you can say something like "could I have some ice please?" or "Can I have a cup of ice please?" These are both easy ways to ask someone for ice and are also polite ways to ask for ice. If you want ice in your drink you can say something like "could you please make sure my drink is iced please.".
