これもそのまま、"Does it exist?" （それは存在／実在するの？）という直訳に近い表現で伝わると思います。 ここに "even” （～さえ）を入れると、（それはマジで実在するの？）という強調した表現になりますね。
より簡単に、"Is it real?” というだけでも全く通じますので、英語はいつでも短く、簡単に言うことが好まれます。この場合でも上記と同じく、"even” を間に挟む言い方もできますよ～！ ”Is it even real?!" なんてね。

This is a casual phrase we use to show our excitement/shock about something. We are usually quite happy about what we are hearing. For example, you go to a beautiful place on holiday and you can't quite believe the beauty of it. You would turn to your friend and say "is this even real?". This phrase can be used for people too!

The imaginary world and the real world are often distinguished by the phrase 'fact or fiction.' If in doubt at any time during a conversation, you may ask the question:
"Are we talking about fact or fiction?" Or, if speaking about a personality, you may say:
"Is that a real person.?"

That all just sounds "so far-fetched!" ...Is that even real?
When something sounds "unbelievable" ...or very far-fetched it usually turns out to be unreal or an exaggeration!
We do have a handy phrase that will sit very well in those circumstances;-D

信じられない、もしくは事実からかけ離れているような話は大抵、本当ではなかったり誇張された話だったりしますよね。そのような場合に使う表現としては以下が適切でしょう。
"Is that even real?"（それって本当なの？）
"That all just sounds "so far-fetched!" ...Is that even real?"
（それかなり現実離れして聞こえるよ。。本当なの？）
When you ask 'does s/he or it exist', you are asking if someone or something is real. We add the 'really' to emphasize that we are speaking of something factually existing.
Same goes for the second expression, you can ask the question as 'is it real?' but we add the 'even' for emphasis.
'does s/he or it exist'と尋ねる時は、人や物が実在するかどうかを尋ねています。話している物が実際に存在することを強調するために'really'という語を加えます。
2番めの例にも同じことが当てはまります。 'is it real?'と尋ねることが出来ますが、強調するために'even'という語を加えます。
In the first two examples we can either use the pronoun, "they," or use, "that person," when trying to verify if the person actually, "exists," or not. We can also use the adjective, "real," to confirm if the person exists or not.
