英語には「ボタンを開ける」という意味の動詞があります。to unbuttonです。これを変形すると「ボタンが開いている」という意味の形容詞になります：unbuttoned.
Wearing a shirt → シャツを着ている
with a few buttons → 幾つかのボタンが
unbuttoned → （ボタンが）開いている
1) the man with his top three shirt buttons undone
「シャツのボタン上から３つを開けている男性」
undo で「結び目をほどく・ボタンをはずす」という意味です。
2) the man with a few buttons on his shirt open
「シャツのボタンをいくつか開けている男性」
ボタンを「いくつか」と言う場合は a few buttons と言えます。
