ご本人様ですか?って英語でなんて言うの?

病院の受付などで、保険証を出した際に聞かれることがあります。 本人なのか、患者の付き添いなのか。もちろん名前がわかっていれば "Are you Mr. / Ms. TANAKA?"と聞けばいいのかもしれませんが、 名前がわからないときなんかは、どう聞くのか気になります。

Are you Mr. / Ms. TANAKA?と聞いてもいいです。経験の中では下記のようなパターンの方が多く聞きましたが 例えば、 Are you the person who made the reservation? 予約されたご本人様ですか? (電話) 田中の予約についてですが はい、ご本人様でよろしいでしょうか I'm calling about the reservation for Tanaka Okay, is this your reservation? Is this reservation for you?もOKです。 保険証を出した際に「ご本人様ですか」は英語で Is this your insurance card?もしくは Are you the owner of this insurance card? (保険証の持ち主ご本人様ですか) と言います ご参考になれば幸いです。

保険証を出しにくる人に、患者さんの名前がわからなくてもこの聞き方はいかがでしょうか。 Are you the person listed on this card? この保険証に名前が書かれている人ですか。 つまり、「この保険証に書いてある名前はあなたの名前ですか」と聞きます。 もう一つの聞き方は「patient(患者)」という単語を使います。 Are you the patient or are you accompanying the patient? あなたは患者様ですか。それとも患者様の付き添いですか。 「Accompany」は「付き添い」という意味ですが、英語だと動詞ですので「ing形」の「accompanying(付き添っている)」を使います。
