「推薦状は、本学様式を使用していますか？」という質問は"Is it written using the format of ABC University?"で伝わるでしょうか？
「本学様式」は"the prescribed form from ABC University"という表現も見つけたのですが・・・

本学とは、特に大学についていう場合が多く、その大学の職員などが自身の勤める大学を指して言う場合が多いですね！したがって、ABC大学様式とも言い換えられます。
『the format of ABC University』で、『ABC大学の様式（形式）』という意味ですので、
Is it written using the format of ABC University? で伝わります。
また、
Is it written in the ABC University format? としてもよいでしょう。

英語では “format” でも伝わりますが、書類のチェックリスト文脈だと format より form / template が自然です。特に「推薦状」は、決まった用紙があるなら form、決まった書き方（ひな形）があるなら template が合います。
