ーDo you want to get on the couch?
「カウチの上に上がりたいの？」
to get on the couch で「カウチの上に上がる」
ーDo you want to sit on the couch?
「カウチに座りたいの？」
ーDo you want to sit up here with Mommy?
「ここに（カウチに）ママと一緒に座りたいの？」
もしお母さんがカウチに座っているなら、このような言い方もできます。
ーDo you want to get off the couch?
「カウチから降りたいの？」
to get off the couch で「カウチから降りる」

Kyokoさんも書いた通り 「Do you want to get up on the couch?」でも全然問題ないです。
また、「降りたいの？」の場合、「Do you want to get down from the couch?」は正しいです。
