A more casual option, which is still respectful, would be "What brings you here today?"
You could also ask, "Are you having any symptoms?" or, "Are you experiencing any symptoms?"

In the first question, you firstly greet the patient and then ask how you can possibly help him/her. You expect the patient to explain what she is suffering from.
In the second question, you are basically asking the patient to tell you how she is feeling. You expect the patient to explain her dental problem.
In the third question, your expectation is the same as that of the second question. You expect the patient to explain her dental problem.
By asking 'what seems to be the trouble?', you are acknowledging that there is most likely a reason involving pain or discomfort that has brought the patient to your clinic. It is fine to say this to everyone, even if they are not in pain and just coming in for a regular check up, as it is expected in a medical facility that a lot of people will be coming to see a medical professional to receive attention for pain or illness.
