Here are three ways you can say Happy Birthday!
1. To a friend, you can say - Happy Birthday my Friend!
2. If you want to mention the person's age, especially a child's age or a milestone age (18, 21, 30, 50, etc.) you would say, "Happy 20th Birthday!"
3. If you are wishing someone you do not know as well, a colleague, client, etc., you can simply say, "Have a Happy Birthday!"

If you know the age it is common to include the number they are turning in your greeting, such as Happy 21st Birthday. A simple 'Happy Birthday' is a suitable greeting for all levels of relationships and formalities. For a very formal option, the phrase "I wish you a [very] happy birthday" is polite also.

When we wish someone a Happy Birthday and want to include their age, we say this:
Happy (age number with --th or -st) Birthday
example, Happy 10th Birthday (10 years old)
Happy 21st birthday (21 years old).
We can also simply just say Happy + age (number -st or -th) and NOT include the word Birthday after. It is assumed that you are referring to someone's birthday when you say this. However, this expression can be used for any special occasions that are celebrated by age or year such as anniversaries.
