私はI should have asked— の疑問文が、Should I have asked—?か、Have I should asked —-?か分かりませんでした。今は前者が正しいと分かっています。その時にこの、I should have asked—の疑問形は、何ですか？と尋ねたかったのですが、その尋ね方が分かりませんでした。教えて頂きたいです。

make this sentence into a question で「この文を疑問文にする」
「この文を否定文にする」なら make this sentence into the negative form となります。
「この文を未来形にする」なら make this sentence into the future tense となります。
2) What's the interrogative form of this sentence?
「この文の疑問形は何？」
interrogative は「疑問の」という意味で、 interrogative form で「疑問形」となります。
