died or was badly affected by the incident/attack/event.

In this case, since the focus is your grandfather, I think that it is important to start the sentence with the fact that he was exposed to radiation. When we talk about dates, we use ordinal numbers which is why it is sixth instead of six. I hope that this helps. :)
