アレルギーや持病をお持ちですかって英語でなんて言うの？

アメリカの病院で一番よく聞く表現は、Are you allergic to any medications?です。
Are you taking any medications (right now)?という質問と合わせて聞かれることが多いです。

持病に該当する言葉は、medical conditionあるいはmedical historyです。
本人だけでなく家族の持病（既往症）について、Do you have any medical history in your family?という質問もされることが多いです。
high blood pressure（高血圧）、cancer（癌）、diabetes（糖尿病）などがあれば伝える必要があると思います。

any + 複数形 だとアレルギーなり持病なり複数持っていることを想定していますが、a + 単数形 だと「何か一つ持ってる？」というニュアンスになります。

「～にアレルギーがある」は have an allergy to ~、be allergic to ~ と言います。
「持病」は chronic disease とも chronic illness とも言います。

Do you have any allergies or chronic illness/diseases?
何かアレルギーや持病はありますか？

Are you taking any (other) medicine?
(他に）何か飲んでいるお薬はありますか？

Be aware that there is quite a big difference between, "allergies," and a, "chronic illness," as the second example is more severe whereas an allergy can be something simple like an allergy to pollen or flower residue. We can also simple ask if someone has any, "medical issues," which is a very broad and general term.
