ご質問の文は英訳例のように2通りに言うことができます。
（1）「子供たちはあなたのことを何と呼べばいいですか」
この文は call 人 〜 で「人を〜と呼ぶ」という call の使い方です。「〜」の部分が分からないので what にし、それが疑問文なので文頭にきている形です。
The children should call you 〜. を元の形と見ると良いです。
（2）「あなたは子供たちにあなたを何と呼んでほしいですか」
上記の call の使い方に、さらに「〜に…してほしい」という want の使い方を混ぜたものです。want + 人 + to不定詞 で「人に〜してほしい」という意味です。
You want the children to call you 〜. を元の形と見て、「〜」に what を当てはめて、、、のように、あとは（1）と同様に疑問文にすればOKです。
また少し丁寧に言う場合であれば、want + 人 + to不定詞 の代わりに would + like + 人 + to不定詞を使うこともできます。この表現を用いた場合には次のようになります。
What would you like the children to call you?

「呼ばせる」は 使役動詞を使います。
①have (あるいは make ） ＋目的語（ここではthe kids あるいは、the children）＋動詞の原形（call）
（この文のhave あるいは make が使役動詞です）
あるいは
② get（使役動詞）＋目的語 (the childrenや、the kids)＋ to ＋動詞の原形(call)
①の場合、make の方が、have より強制的に呼ばせるという感じです。
②の get と to ＋動詞の原形を使う言い方は、イギリス式です。

When we speak / write or email a person... we are still addressing them:-)
They may want us to use their title or call them Joe...Its a personal choice!

a polite question, using "what would you like" is often used in formal and polite situations.
