「味や匂いは普段どおり感じますか？」
normally を入れると「普段と比べてどう？」が自然に含まれます。
患者さんが短く答えやすいので、最初の質問に向いています。
“変化” をストレートに聞く（ニュアンスがズレにくい）
"Have you noticed any changes in your taste or smell?"
「味覚や嗅覚に変化はありましたか？」
noticed（気づきましたか）＋ changes（変化）にすると、「少し変」「戻ってきた」などの細かい説明が出やすいです。
or にするのは、味だけ／匂いだけの変化も拾うためです。
