A: 朝食は普段、何を食べていますか?

If you use the adverb 'usually' as used in the first line, you mean to say that generally you skip breakfast, that is, most of the time. But, if you used the adverb 'always' as in the second line, you mean to say that you miss breakfast all the time. The third line is just the same as the second line. You are saying succinctly that you do not eat breakfast at all, that is 'always', but you eat a large lunch instead.
So, you may say:
I usually skip breakfast and eat a large lunch instead.
or
I always miss breakfast but eat a sizable lunch instead.
or
I do not eat break fast but I eat a large lunch instead.
一番初めの例文にあるように、"usually"という副詞を使うなら、ほとんど通常朝食は抜きますという意味になります。
しかし二つ目に例文で使われているように、副詞"Always”を使うことにより、いつも朝食を食べませんという意味になります。
三つ目の例文は二つ目の例文と同じ意味になります。簡潔に朝食は”always"食べないが、その代わりにランチを沢山食べますと言っています。

以下の例文を使えばいいでしょう。

I usually skip breakfast and eat a large lunch instead.

I always miss breakfast but eat a sizable lunch instead.

If you 'skip' a meal it means that you do not eat when most people will be eating any one meal. If you eat 'heartily' it means that you have a very good appetite and consume a good quantity of food by normal standards. Usually, the first meal of the day is breakfast so any deviation from that is considered a little unusual.
 'skip' a meal とは、食事を抜くという意味です。eat 'heartily'とは、食欲が旺盛で普通よりもたくさん食べるという意味です。たいていは一日のはじめの食事は朝食ですが、そうではない人もいるようです。
