When someone asks 'What's new?' They want to know how you are doing and if there is any new event in your life. It is a way of starting a conversation.
Another way is to say: How are you doing today?
The best way to answer is to talk about a recent event or in case there is nothing new you can say:
'Not much'
'Nothing much'
That means that there has not been any unique events in your life recently.
If things are pretty much the same as when you last spoke you can say:
'Same old'
If you have something to say you can begin by saying:
Well actually, last week..
Well actually, today I...

What's up? や What's happening?と同じような表現で、最近何が起きたかを聞くことにより会話を開始する表現の一つです。
How are you?などの表現と似ていますが、「あなた」が現在どうなのか聞いているのではなく、「何が起きているか」を聞いているため "I'm good"や"I'm fine"のような返しは間違いとなるので注意してください。これは間違えてしまう人が多いです。
回答としては、とりわけ何か思い浮かばない場合は

と答えるとよいでしょう。
Same oldは２回続けて言うことが多いです。
