We use both of these phrases quite regularly. It is always polite to ask do/would you mind, when asking for a favour. It is a good sentence starter for when you approach a stranger or even a friend. People are not always so open to having photos taken of them so politeness is key in these situations.
これらの表現は両方ともよく使われます。何かをお願いするときは、do/would you mindを使うと丁寧な表現になります。見ず知らずの人でも、友達でも失礼なく頼むことができる表現です。必ずしもみんなが写真をとられるのを気にしないとは限りませんので、丁寧に尋ねるというのがこの場合、カギとなるでしょう。

We use the expression, "Do you mind if...." when we want to ask someone very respectfully if we may do something. We are effectively, asking for permission to do something. A: "Would you mind if I took this chair." B: "Sure, no-one's sitting there." Take a pic/take a snap = take a photograph

"Would you mind if he takes a picture with you?" is a polite request asking if someone could take a picture with another.

All the sentences are a good way to ask if you can take a photo of your child with the teacher By using the words `can i/he/she' or `would you mind' it is a nicer way of asking if it is possible to do something and by adding please it is a polite way of saying it as well

You can ask straightforwardly and say: Can my child take a picture together with you? You may also politely ask the teacher to allow your child to take a picture with him/her. Asking politely will do wonders as it is always the accepted norm. So, you may say: Can my child take a picture together with you? or Please allow my child to take a picture with you.
