1) How long have you been waiting?
"have you been waiting"は、現在完了進行形なので、その時までずっと待っている状態が続いていた時に使います。例えば、待ち合わせで待たせた相手に会ってすぐ言う場合はこちらです。
2) How long have you waited?
”have you waited”は、現在完了形なので、待っていたという事実に対して使います。
3) How long did you wait?
”did you wait”は、過去形で、待っていたその時のことを聞くのに使います。"yesterday"などある一点の過去を表す言葉と一緒に使う時はこちらです。
待ち合わせ場所ですぐ「どのくらい待った?」と聞く時は、1)を使い、それ以外は2)3)でいいと思います。
