謝る必要があるかどうかは別として、「Sorry my grammar is all over the place」を使えます。
こちらはイギリスのスラング的な言い方で、「all over the place」はめちゃくちゃと言う意味ですので、このシチュエーションには適していると思います。
もっと平たく言うとしとしたら、「Sorry my bad grammar」があります。
外国人の人は質問者様の文法がめちゃくちゃでも、指図しないと思いますよ。
彼らに日本語を話してと言っても、話せない人の方がいるので、
頑張って外国語を学ぼうとしている人に対して悪く思う事なんて普通ではありえない話です。
まぁ、日本の英会話学校を経営している陣に言わせてみれば、文法は大事なのでしょうけどね。文法がパーフェクトになってしまったら、彼らのビジネスは成り立たないので。さらに言えば、多くの英会話講師は文法を指摘する事で食べているので、存分に指摘する講師も中にはいるでしょう。
僕自身、文法にそんなに気にしません。
頑張って英会話に時間を注げば必然的に上達するものですので、時間と訓練の問題だと思います。精神論ですいません。
"I'm sorry my grammar is incorrect." This phrase explains how your grammar is not right.

poor という単語は貧しいとかやせたという意味で使われることも多いですが、ここでは乏しい、不十分、標準以下というような意味合いです。

（英語がたどたどしくてすみません）
broken English で片言の英語,下手な英語という意味です。broken はbreak(壊す、切断する、折るなど）の過去完了形で、ここでの意味としては半端の、デコボコのある、はちゃめちゃなと意味で使われています。 スムーズに話せない、英語が完璧でないというニュアンスを伝えることができると思います。
またapologizeという単語は頭を下げる、詫ぶ、詫びる、謝罪するなどの意味があり sorry に比べると少しかしこまった言い方ですので、お友達間で話すときはこの部分をsorry に置き換えて使うといいかと思います。

"Isn't that well" means that it is not good but not very bad as well.
"at a high level" means a fluent level which is achieved by studying every day.
a mess: is another way of saying poor grammar or not very good grammar but in a more slang kind of way. It is still acceptable to be used in conversations and writing, if you are sending messages.
These phrases show that you lack some knowledge of grammar but altogether, your grammar can still be understood.
