「先生」とお医者さんを呼びかける時には Doctor と表現します。
cause は動詞でもあり、名詞にもなります。
1番目は「何が原因ですか?」という意味になり、what が現在形三人称単数扱いになりますので、cause に s が付きます。
2番目の cause は 名詞ですので、what is the cause(その原因(単数形)は何ですか?)と原因は1つに絞って聞いています。 What are the causes...? という聞き方もありです。 その場合は日本語では同じく「原因は何ですか?」ですが、原因を1つではなく複数の原因を知りたい場合に使います。
