相手がいきなりいいねと言ってきたけど自分の何についていいねと 言ってるのか分からないと言う状況でしょうか。 その場合は直訳で regarding my smell と聞くのが良いと思います。 私の匂いがって言う事と聞く感じで後者の you mean my smell と言うのもおすすめです。

Are you saying that because of how I smell? シンプルで自然なのはこの2つです。 ・Do you mean the smell? （匂いのこと？） mean は「〜のことを言ってる？」という確認です。短くて会話でよく使います。 ・Are you talking about the smell? （匂いのことを言ってるの？） talk about は「話題にしている」なので、相手の発言の対象を確認できます。
