NARIさんの文はほとんど会っています。もっとネイティブな言い方になるために "Please" を一回だけ言います。
"tell someone at the reception" の後に "that" をつけます。 "tell someone that ~" のパターンです。
そして "done" の代わりに "finished" の方が適切と思います。
"examination" も "your examination" 「あなたの検査」になります。
"Please go back to your department, and tell someone at the reception that you've finished your examinations."
Please go back to the Internal medicine desk, and tell them you finished doing your tests.
例えば、診療科が内科だった場合、次のように言うと良いでしょう。
