アメリカ人は「Toilet」をあまり言いません。代わりに「bathroom」と言います。
おっしゃった「お手洗い」に合うと思います。
この場合、「誰かがお手洗いにいる」と言うので、
Somebody is in the bathroom.
Someone is in the bathroom.
自然な発音はちょっともごもごするので、ネイティブはよく「サンバディー・イズ」ではなく「サンバディーズ」と発音します。

If no one is in the toilet, it is vacant
Public toilets in England usually have a sign on them which says either Engaged or Vacant
もし誰かがもうトイレにいるのなら、It is engagedといいます。

When talking about a toilet or bathroom we often use the word restroom as a more polite way of saying this
if someone else is in the bathroom already then you can simply say 'somebody else is in the bathrom'
occupied also means that something is already being used by someone
