"spaced out" - this means that your mind is distant, as distant as the stars in space. You can't concentrate. "feeling out of it" - this means that you feel absent from everything that is happening around you. You're missing out on a lot of what is going on.

A very high temperature refers to a fever. Also, you can just say you aren't feeling well at all. When your mind wanders and you are not really "present" in your current situation you can say you're spacing it out. You are not really focused on what's happening here and now.
とても高い熱と言うなら、feverです。また、I'm not feeling well at all.(体調が全然よくありません)と言えます。 集中できなくて、今の状況にあなた自身が存在してないなら、I'm spacing it out(ボーッとしてる)と言えます。実際に、ここで今起きていることに集中してないということです。
