The first example, 'cancellation fee', is the most common and correct. This is because the fee you are charged has only been levied because you have cancelled your booking.
To terminate a contract, is to end it. So you could also call it a termination fee.
As most cancellation fees will be written into the contract you enter into when making the booking, this could also be known as a 'contract fee'.
The first two examples are most commonly used. The last example can be used as a noun or a verb, so you can say, for example:
