(1) a baby can hold up it's own head
Hold だけは「持つ」の意味になりますが、「hold up」と言いますと,「支える」の意味になります。
例えば
Look at Sachi. She's three months old and has started being able to hold up her own head.
サチちゃんに見て。もう３ヶ月になって、自分の首をすわるようになった。
(2) a baby that can lift up it's own head
「lift up」と言うのは「持ち上げる」の意味です。
例えば
Lift up both your arms.
両手を持ち上げます。
Can your baby lift it it's own head yet?
