断腸の思いって英語でなんて言うの？

「断腸の思い」は英語で　heartbroken thoughts / heartrending grief / overwhelming sorrow​ などで英訳出来ます。 一番自然な英語は場合によって異なることもありますが、「大変重い決断の上での行為」というような文脈なら〜 "It is with the deepest regret/sadness that ... " 又は "It deeply pains me to (have to) do this, but ... " が一番ふさわしい表現になると思います。 ご参考になれば幸いです。
