It's better to say 'did you understand that okay?', 'did you understand that easily?' or 'did you understand all of that?' than just saying 'did you understand?' as without adding okay, easily or 'all of that' on the end, it can sound quite blunt, and almost rude. Adding 'easily', 'okay' or 'all of that' on the end completely changes the tone of the question, into something friendly and it makes you sound approachable and caring.

When asking someone to tell you that they understand what you have explained you can say "Do you understand?" Or you could also say "Do you get it?" which means do you understand.

After a long and complex explain it is common for the speaker to ask the listen if he or she understood what was explained. When asking questions such as: Do you understand? Does this make sense', etc. The speaker is asking for confirmation from the listener as to whether the message was clear and understood. If not, the speaker would then have to repeat the message or rephrase the content of the information given.
'Are you following me?' is not to be taken literal in this context. The word 'following' in this case means to listen and understand what is being said.
