1)toはいりません。go somewhere/anywhereで「どこかに行く」の意味があります。どちらも使われています。 2)よく週明けにかわされる日常会話で、 「週末どうだった?どこかいったの?」的意味で訳すと、 「あなたの週末はどうだった?」「どこか行ったの?」 私の経験的では、somewhereで聞くと「どっか行くのが普通だよね~」 anywhereで聞くと「行かなかったかもしれないけど、もしかしてどこか行ったの?」 というニュアンスを感じます。

All these would be good to ask ..... if you ask if they done much it means did they do alot... To get upto means what did they do....... you could also ask if they went away at the weekend meaning to leave home to go somewhere different

These three sentences are the most typical way of asking this question. There are no differences in terms of meaning, only the way of asking. For example - How was your weekend? Oh, thank you, it was good!
