と言う会話はよく耳にしますね。
In the leadって言い方は小慣れた感があるので覚えておくといいです！
"Who"はもちろん「誰？」「どなた？」の意味がありますが、このようにチーム名を表す際にも"Who"を使うことができます。
Who's up by how much? ＝どっちがどれくらいで勝ってる？
"how much"だけだと「いくら？」と値段や数量を訪ねる時に使いますが、ここに"by"をいれることによって「どれくらいで？」という意味になります。
また、この場合の"up"は「勝っている」という意味になります。
「何点差で勝ってる？」と聞きたい場合は "by how many points?"と言ってもOKです。
Who's on top = which team is winning/which person is in the lead
If it is a game that includes scoring goals or points, you may ask:

The question 'which team is poised to win?' is the same as asking as to which team is playing more aggressively that they may score more points or goals and win the match.

"What's the score?" - You can ask the person what the score is to see what team is winning or which team is losing.
