How long have you been in Japan? は「日本に来てどの位経ちますか？」という意味です。
この文では "have you been" （現在完了）を使っているので、ある過去の時点に日本に来てからの期間を聞いていることがわかると思います。
もしこの先どのくらい滞在する予定かを聞きたいなら、

The third phrase is a more passive form of question, and uses the less formal word "visit" to describe the trip.

You want to ask someone how long they are going to stay in Japan. (For example someone who is here for school). Of the above two questions, the first is probably the most typical way of asking this type of question. The second example shows an alternative way.

P.S. 前置詞で（staying FOR...など)、文を終わることが間違い、「正しくない文法」など、と主張している人がいますが、実際には大勢の話し手により使われている表現だから、実は間違いとは言えません。
