「ずつ」＝ each もなりますが，英語で「each」１回のみ言いますので，「two each」になります。

Just order two of each item - it will save any possible confusion - ie use the second suggested example above. The first is logical and makes sense but it would be an unusual way of ordering.
If the person knows what dishes you want then you can simply say 'can I have two of each please?'
If you have a menu or can show them what you want then you could say 'Can I get two of these please?' or 'can I get two of this please?'

"Can I add two extra?" works when you are ordering individual items that are countable. For example, when ordering fish and chips that normally come with two pieces, asking this question will get you four pieces.
For dishes that are not countable like pasta or soup, asking "Can I get a double order?" will get you a bigger portion.
If you just want more of something, you can also ask "Can I order extra?" For example "Can I order extra mashed potatoes?" or "Can I order extra french fries?"

It's always nice to say 'please' when requesting something at a restaurant. You might use the word 'these', like in the second example, if you were actually pointing to the items on the menu.
