話題の場合だったら「bring up」になります。
自分からその話題を持ち出した。

Proactively は[積極的に](https://eikaiwa.dmm.com/uknow/questions/44750/)、自ら[行動をする](https://eikaiwa.dmm.com/uknow/questions/51122/)ことなので 「自分から」はいらないです。Approach は日本語の「アプローチ」と少し違い、「寄る、話しかける」という意味です。
Start a conversation / strike a conversation = 会話する、話し始める。
又、「この習慣を実行しています」は英語で “making a habit of 〜” / “trying to regularly 〜”
