Are you mad at me?
これで「怒っていますか？」と言えますが（間違ってはいません）これはスラングです。
もともと mad は「気の狂った・無分別な」という意味なので、
Are you angry at me?
と言った方が正しいです。
Are you mad with me? とは言いません。
with me を使いたいなら
Are you angry with me?
と言います。意味は Are you angry at me? と同じです。
