The most direct way to tell someone to stop following you is to simply say "Stop following me!"
If you are especially angry at this person, using the word "please" is not necessary but if you are trying to be polite to somebody who might not understand that they are bothering you, saying "Please stop following me."
"Leave me alone" is another way to tell someone that you don't want them to be near you. It can be used in many situations, such as when you don't want someone to talk to you or to be in the same place as you.

Because this is an uncomfortable and sometimes scary situation, it is okay to be informal. However, you can still use the word 'please' when stating your demand or request. If you are being followed by someone you do not know, then you can tell them to stop following you and let them know that you will call the police if they do not go away. If you are being followed by someone you do know, then you can simply tell him/her that you want them to stop and to leave you alone.
